To Whom It May Concern;
Mayo Clinic is conducting a survey to see how you feel about wearing a mask for Coronavirus (COVID-19) prevention. Based on what we find out, we will try to help meet the needs for information and masks.
We are seeking volunteers ages 18 years or older who live in Southeastern Minnesota.
Would you be willing to take our survey?
The survey will be available until August 19, 2020.
